Medical Needs

If your child has a medical condition which requires the administration of medicines at school, then please read the 'Medical Information Letter' at the bottom of this page and complete the form with all the details requested. These forms must then be returned to the school office.

Please see our Medical Policy for more information.

Where a child's medical needs change, it is important that parents update us as quickly as possible in order to ensure that we can accurately meet these needs whilst they are in school.

Please do not hesitate to contact us if you need any assistance in completing the form, or to notify us of any changes.

Starting a new school year

On the first day of term, there will be a desk on the front playground for you to hand in your child's medication and completed form. Without a completed form, we cannot administer medication.
Please do not hesitate to contact the school office if you need any assistance in completing the form.

If your child is in year 6, medication in school will be returned to your child on the last day of the school year. Otherwise medication that is in date will be kept in school over the summer break.
